Eat Smart Move More Coalition

The Eat Smart Move More Coalition was founded in 2008 in response to the award of an ACHIEVE (Action Communities for Health, Innovation, and Environmental ChangE) to the Cleveland County Health Department and the Cleveland County Family YMCA.  Funding for this grant originated from the Centers for Disease Control and Prevention, YMCA of the USA and the National Association of Chronic Disease Directors to improve health through environmental and policy changes.  Cleveland County was one of ten communities nationwide in 2008 to receive funding from this program.

The ESMM Coalition was established to serve as a community forum to guide and direct programs and policies in the areas of nutrition and physical activity especially as these factors affected chronic disease.  Composed of 25+ community volunteers representing schools, businesses, agencies and individuals, the coalition focuses its efforts on responding to identified issues surrounding healthy food choices and physical activity.  Successful programs include the development of identified walking trails in Shelby, Kings Mountain and Fallston, support for the Foothills Farmers’ Market – especially the satellite market held at the Public Health Center, development of guidelines for healthy foods and physical activity for afterschool programs and business/agency meetings.

Today the ESMM Coalition is focused on full implementation of the Live Healthy Cleveland County web site as the one-stop shop for individuals who are seeking healthy foods and greater opportunities for physical activity across the county.   Coalition members also support the goals and objectives of the NC Healthy Communities program in addressing tobacco issues, promoting healthy lifestyles and preventing chronic diseases.

The goal of the Catalyst is to spark transformational change, creating places where healthier choices are available and easier to make. Catalyst Coordinators work to connect partners and strengthen efforts for healthy living. Their work focuses on creating places and systems that make it easier for people to make healthy food choices and be active. The Catalyst Coordinators are there to:

  1. Connect leaders and nontraditional partners to build momentum for healthy living;  
  2. Engage community members to integrate local knowledge and resources throughout planning and action aimed at making healthy eating and being active easier;
  3. Coordinate with local and regional initiatives that will have long-term impact for residents across the county.

The Catalyst envisions empowered rural communities that acknowledge health inequities and advance change to improve health and wellbeing for everyone.

The Catalyst is a program of the Foundation for Health Leadership & Innovation and is implemented in partnership with the North Carolina Division of Public Health and with funding and support from the Kate B. Reynolds Charitable Trust.

The Cleveland County Public Health Center was organized as a local health department and has a long history of providing public health services in response to the need for prevention and control of communicable diseases.  The mission of CCPHC is “to assure, enhance, and protect the health of Cleveland County citizens through education and prevention.”  Today the department is a multi-disciplinary organization addressing health issues through provision of direct services through its clinics, through education provided to a variety of audiences in the county and through community partnerships such as coalitions and task forces working to resolve specific health issues like substance abuse and teen pregnancy.  With a talented staff, a well-designed facility opened in 2016 and a supportive Board of Health, the Cleveland County Public Health Center has stepped into the 21st century of health care ready to meet the challenges of an ever-changing health environment.
